Documents to Submit at Admission - Registration Guidelines Documents Needed

• Government-issued Photo ID Proof (Aadhaar/Passport/Voter ID)
• Photo
• Insurance Card / TPA Authorization Letter
• Pan card (policy Holder)

At the Admission Desk

• Submit the above documents at the Admission Desk
• Admission consent & Consent for insurance treatment will be filled and signed
• Room will be allotted based on preference and availability

Already Have Soft Copies?

• You may email (insurance@sutpattom.com) the scanned documents to the insurance team
